Geographical Restrictions:

All patient encounters, whether they are in-person or virtual (including live synchronous telehealth or asynchronous telemedicine), will be exclusively conducted with patients who are physically located within the states where the medical provider holds an active medical license.

Peak Integrative Health, LLC will maintain an up-to-date list on our website detailing the states in which our medical providers are currently licensed to practice. This list will be updated promptly with any changes in licensing status.

Patient Notification:

At the time of booking an appointment, patients will be informed of this policy to ensure they are aware that they must be located within a licensed state for the duration of the medical encounter.

This policy will be reiterated at the initiation of each patient encounter, both for virtual and in-person consultations, to confirm compliance.

Non-Compliance and Refunds:

There will be no refunds provided if a patient fails to comply with this policy. It is the responsibility of the patient to ensure they are within a state where the provider is licensed during the time of the medical service.

Indemnification:

Patients agree to indemnify and hold harmless Peak Integrative Health, LLC, its employees, officers, agents, and medical providers from any liability, loss, or damage resulting from providing false or misleading information regarding their location at the time of the medical encounter.

Compliance:
All staff members, including administrative and medical personnel, are required to enforce this policy rigorously to ensure compliance with state and federal laws regarding medical practice jurisdiction. Patients found to be non-compliant will be advised to reschedule once they are within a licensed state.
Effective Date:
This policy is effective as of 1/1/2024 and will be reviewed annually or upon significant changes to state licensing laws or company operations.
